HOSPITAL – Clearwater, FL 32 hrs/weekly Social Services Specialist II – Emergency Department & Med-Surg floors ( From 2011 – Jan 2012) – OB/Labor & Delivery – from Jan 2012 – current. The Social Service Specialist II is a member of a collaborative practice model that includes patients, nurses, clinical resource coordinators, physicians, and other practitioners, caregivers, and the community. Knowledge about disease, treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs. Goals include achievement of optimal health, access to care and safe transition of care through the continuum balanced with the patient’s right to self-determination. Provided case management services children and their families. This included home visits, family centered referrals, advocacy and mentoring intervention to high risk families and children. Wrote court ordered case plans, reviews, and social study reports for the dependency court. Assisted state attorneys with affidavits, motions and complaints for juvenile court. Interacted with all parties including defense, prosecuting, DJJ partners and guardian ad litem attorneys to provide the most effective intervention for the child. Fully trained in the Family Finders Program – Assisted in locating missing family members of adolescents lost in the system.

EDIP II - 01.2005 until 10.2006 - 1 years and 9 months

Case Manager Worked exclusive with trouble teens involved with DJJ. The program was responsible for evaluating adolescents for early risk of offending and reoffending and prevention of delinquency. My responsibilities included: develop and monitor the teen’s case plan and their compliance with the tasks and courts; weekly home visits, random drug screens, school visits, family assessments, writing weekly reports for the courts, and court appearances.
